You can download the form below and complete the application. Then, simply fax it to 780.998.4774 or drop it off at City Hall, 10005 - 102 St., Monday to Friday 8:00 a.m. - 4:30 p.m.
What happens if I forget my password?
You can request your password by supplying us with your existing user name and email address. Once your request has been verified, an email will be sent to you with this information.
